A study from Finland, conducted through the Nordic Research Network in Tampere, has found that a next-generation hepatitis B vaccine offers dramatically stronger protection for young adults than the standard single-antigen shot currently used in the United States. For Americans living with diabetes—a group at significantly higher risk for hepatitis B infection—this research carries real and urgent relevance.
What the Researchers Did
In this large Phase 3 clinical trial published in JAMA Network Open, researchers across 37 clinics and hospitals in Canada, Europe, the United Kingdom, and the United States compared two hepatitis B vaccines in nearly 2,838 healthy adults aged 18 to 45. One group received the newer 3-antigen (3A) hepatitis B vaccine, and the other received the standard single-antigen (1A) vaccine—the type most commonly given in the US today. Participants were followed for 48 weeks after their first shot.
What They Found
The results were striking. After just two doses, the 3-antigen vaccine provided seroprotection (meaning enough antibody levels to be considered protected) in 90.4% of participants, compared to only 51.6% in those who received the standard vaccine. After the full three-dose series, protection rose to 99.3% with the 3-antigen vaccine versus 94.8% with the standard shot.
Antibody concentrations—a measure of how strongly the immune system responded—were 7.9 times higher after two doses and 3.5 times higher after three doses with the 3-antigen vaccine compared to the standard option.
The 3-antigen vaccine did cause more local reactions like arm soreness (85% vs. 66%) and general side effects like fatigue (68% vs. 60%). However, serious adverse events were rare in both groups, and very few participants stopped their vaccine series due to side effects.
How This Compares to US Guidelines
The CDC and American Diabetes Association (ADA) currently recommend hepatitis B vaccination for all unvaccinated adults with diabetes under age 60, and it should be considered for those 60 and older. The standard 3-dose single-antigen Recombivax HB or Engerix-B vaccines have long been the go-to options. However, immune response rates in adults—especially those with diabetes, who often have compromised immune function—can fall short. The 3-antigen vaccine (Heplisav-B is one available 3A-type option already FDA-approved) may offer a meaningful upgrade.
Why This Matters for US Patients
People with diabetes face up to twice the risk of hepatitis B infection compared to the general population, largely due to shared blood glucose monitoring equipment and frequent healthcare contact. Despite this elevated risk, many adults with diabetes remain under-vaccinated or incompletely protected. A vaccine that provides 90% protection after only two doses—versus barely half with the current standard—could be a game changer, especially for patients who struggle to complete a three-shot series.
